How it works

Most referrals reach us electronically from the hospital before you’re even home.

The hospital refers you

When you are discharged, the hospital sends us details of your stay and any medicine changes, securely and electronically.

We review the changes

Our pharmacist compares your new medicines against what you were taking before, and checks your first prescription after discharge extra carefully.

We talk it through

We make sure you understand what has changed, what has stopped and why — and answer any questions. It keeps you safe and out of hospital.

If you or someone you care for has been discharged in the last few weeks and your medicines have changed, let us know — even if you are not sure whether the hospital referred you. Our pharmacist can check and make sure nothing falls through the cracks.
