Sore Throat: When You Need Antibiotics — and How to Get Assessed Same Day

Here's the uncomfortable truth about sore throats: the great majority are viral, and no antibiotic on earth will touch them. But a minority — most often strep infections — genuinely benefit from antibiotics, and telling the two apart used to mean getting a GP appointment.

The FeverPAIN score, explained

FeverPAIN gives a point each for: Fever in the last 24 hours, Purulence (pus on the tonsils), Attending within 3 days of onset, severely Inflamed tonsils, and No cough or cold symptoms. Score 0–1 and a bacterial cause is very unlikely — antibiotics would do nothing but cause side effects. Score 4–5 and a strep infection is likely enough that immediate antibiotics are appropriate. In between, it's a judgement call your pharmacist talks through with you.

What actually helps while it heals

Paracetamol or ibuprofen for the pain, warm salt-water gargles for adults, plenty of fluids, and lozenges or ice lollies to keep the throat moist. Most viral sore throats are gone within a week. Medicated throat sprays can numb severe pain — ask us which suits you.

Red flags — get help today

Difficulty breathing or swallowing your own saliva, being unable to open your mouth fully, drooling, a muffled 'hot potato' voice, or one-sided severe swelling can signal quinsy or a deeper infection needing urgent treatment.
