Fexofenadine is often the antihistamine people move to when cetirizine or loratadine has not worked or has made them sleepy. It is the least likely of the three to cause drowsiness, which is why it is favoured by people who drive or operate machinery for a living.

What it is used for

Hay fever and allergic rhinitis in the 120mg strength, and chronic hives (urticaria) in the 180mg strength, which is usually prescribed.

The 120mg tablet is available from pharmacies for adults and children over 12 for hay fever symptoms.

How to take it

Usually one tablet once daily. Take it with water rather than fruit juice — apple, orange and grapefruit juice reduce how much is absorbed, potentially by a substantial amount. Leave a couple of hours either side.

It can be taken with or without food. It works within about an hour.

Do not take an antacid within two hours of fexofenadine, as this also reduces absorption.

Side effects

Headache, drowsiness (less common than with other antihistamines), dizziness and nausea are the ones most often reported, and are generally mild.

Get urgent help for swelling of the face or throat, difficulty breathing, or a severe rash.

Who should take care

Anyone with kidney or liver problems, or heart rhythm problems, should check before use. Older adults should also ask, as they may be more sensitive to side effects.

There is less information about fexofenadine in pregnancy than for loratadine or cetirizine, so one of those is usually preferred — speak to us.
